Is gain/loss realized or unrealized?

Unrealized; it's the paper gain or loss on what you currently hold, based on the latest price or NAV. See “NAV updates and realized vs. unrealized gains” for how mutual funds track both kinds.

Does the allocation chart include cash?

No, it only covers your investment holdings (stocks, crypto, other, mutual funds), not bank account balances.
